One minute you’re fine. The next, your heart is slamming, you can’t catch your breath, your hands are tingling, the room feels unreal, and every cell in your body is screaming that you’re about to die or lose your mind. Then, ten or fifteen minutes later, it passes — leaving you shaken, exhausted, and dreading the next one.

If that’s you, here’s the first thing to know: a panic attack, as horrifying as it feels, can’t physically hurt you. It’s your body’s alarm system misfiring — a full fight-or-flight surge with no tiger in the room. That doesn’t make it “not real.” It makes it a pattern — and patterns can be changed.

What’s Happening in a Panic Attack.

A panic attack is a sudden wave of intense fear that peaks fast — usually within about ten minutes — and comes with a pile of physical symptoms: pounding heart, shortness of breath, chest tightness, dizziness, sweating, shaking, numbness or tingling, and a surreal “this isn’t real” feeling. Your body has flipped the panic switch and dumped adrenaline as if you’re in mortal danger.

Panic disorder is what develops when the attacks keep coming and you start living in fear of the next one — avoiding places, situations, or sensations that might set one off. That fear-of-the-fear is the engine that keeps panic disorder running, and it’s exactly what good treatment takes apart.

Knowledge Nugget: The scariest part of a panic attack — “I’m dying / going crazy / losing control” — is your brain misreading normal alarm sensations as catastrophe. Learning to re-read those signals is a huge part of getting better.

When Weekly Therapy Isn’t Enough.

Panic responds really well to the right therapy — and for many people, weekly sessions do the job. But when the attacks are frequent, or avoidance has already taken chunks of your life, more support, more often, gets you traction faster. An Intensive Outpatient Program (IOP) gives you several sessions a week — skills, exposure work, and group — while you keep living your life. Signs it might be time:

Panic Treatment FAQ.

  • A panic attack itself isn’t physically dangerous — it’s a false alarm, not a real emergency. That said, some symptoms overlap with heart problems, so if you have chest pain or trouble breathing you’re unsure about, get it medically checked. Once a medical cause is ruled out, panic is very treatable.

  • No. We treat panic and anxiety on their own. If you’ve been using alcohol or benzos to cope, we can treat both together.

  • Panic is one of the faster-responding conditions — many people see real change in a matter of weeks with focused CBT and skills work. Timelines stretch when trauma or substance use is also in the mix.

  • Exposure work is part of what makes CBT effective, but it’s done gradually, with your consent, and only after you have skills to handle it. Nobody gets thrown in the deep end. We work out the pace together, based on your clinician’s read of what you’re ready for and the skills you’ve built so far.

  • Often yes, but it depends on the medication and your goals. We don’t prescribe — we coordinate with your prescriber. Benzodiazepines in particular can keep the panic cycle going, so we’ll look at that carefully and never change anything without your medical team.
