What "Covered" Means with UnitedHealthcare

Federal parity law (the Mental Health Parity and Addiction Equity Act, or MHPAEA) requires UnitedHealthcare to cover behavioral health on the same terms as physical health. In plain terms: your mental health and substance use treatment is a real, protected benefit — not an optional add-on.

For our programs, UHC coverage typically includes:

  • Intensive Outpatient Program (IOP) — structured day or evening treatment (clinically, ASAM Level 2.1), about 10 hours a week, while you live at home or in sober living. Explore the IOP program.
  • Outpatient Program (OP, ASAM Level 1.5) — a lighter step-down, about 4 hours a week, as you stabilize. See the Outpatient program.
  • Mental health care — with or without substance use — depression, anxiety, trauma, and other conditions. You do not need an addiction diagnosis to attend our IOP; we treat mental health on its own, too. Conditions we treat.

Does UnitedHealthcare Require Pre-Authorization for IOP?

Often, yes — though not every plan does. Many UHC plans require prior authorization — a quick medical-necessity review — before IOP begins, and then periodic re-authorizations to keep it going. Those reviews run through Optum, UnitedHealthcare's behavioral-health administrator. The good news: we handle that paperwork for you, so your care is not interrupted and you are not chasing approvals.

UnitedHealthcare Plan Types — and What They Mean for You

UnitedHealthcare offers several plan types — Choice, Choice Plus, Options PPO, and Navigate among them. The main difference for you is referrals and network rules: Navigate and other gated plans may need a referral and stick closer to in-network providers (that's us — Awkward Recovery is in-network with UnitedHealthcare), while Choice Plus and PPO plans usually offer more flexibility. You do not have to decode this yourself — we read your plan when we run your benefits.

UnitedHealthcare IOP Coverage — FAQ.

  • Yes — most UHC plans cover IOP (ASAM Level 2.1) as a behavioral-health benefit. The covered amount depends on your plan, which we verify for free.

  • Yes. UHC covers dual-diagnosis care — a mental health condition and substance use treated at the same time. You do not need a substance use diagnosis to attend, though: we also treat mental health conditions like depression, anxiety, and PTSD on their own.

  • It depends on your deductible and copay — there is no single price. Once we run your benefits, we give you a clear, specific estimate up front, before you owe anything.

  • No — Optum (United Behavioral Health) is UnitedHealthcare's behavioral-health administrator. It's normal for your mental health and substance use benefits to run through them, and we verify with Optum directly.

  • Many UHC plans still cover part of out-of-network care. We check your out-of-network benefits and lay out your options — including what each would cost — so you can decide with full information.
